Are these measured for single wrap or double?

Hi Joe. If you start with the middle of the belt on your midsection, you can wrap both sides to the back and then to the front again.

If you start with one end on your midsection, you can wrap the length of the belt around your body twice.

This is assuming the wearer gets the proper size for their body type.
